I confirm to be an adult, at least 18 years of age, allowed to receive the medication(s) I am requesting. I am competent to utilize the services offered by, and fully understand the material contained on this website.

I confirm the medication(s) are solely for my own personal use, and will not distribute, sell, or otherwise dispense the medication(s) to any other person. I do not request the medication(s) in order to provide or add to a stock of such. The medication(s) I order do not exceed the amount necessary for my current medical needs.

I confirm I have enough knowledge of the medication(s) I order, either from a local physician or from knowledge from Internet. I confirm I have had a recent satisfactory and sufficient physical examination, and will contact a local physician in case I have any difficulties, possible complications, or questions.

I have previously used the medication(s) requested, under a physician's supervision, without any adverse effect, or I have been advised by my examining physician that I can safely use the medication(s) requested.

I confirm all the questions in the medical questionnaire is answered correct and answered truthfully.

I have fully and completely disclosed all information concerning my health and medical history that might possibly be relevant to my current conditions and need for medication.

I agree to monitor my blood pressure at least once every 14 days. If my blood pressure is over 140/90 (either the top number is greater than 140, or the bottom number is greater than 90) I agree to stop taking the medications immediately.

I am permitted to use the credit card used to order medication(s) from HomePharma. I agree that I am responsible for all customs, tariffs and taxes, if applicable to my country. The laws of jurisdiction in which I reside, permit me to receive and use the medication(s) requested for my personal medical needs. HomePharma is unable to return or issue refunds for any order due to the fact that this is a prescription medicine.

I understand that no doctor or nurse, or administrative personnel can guarantee that the medications, even if prescribed, will provide the results I seek. Further, I understand that even if prescribed, I may suffer adverse effects from these medications.

I am requesting that a European licensed physician act only in an adjunct capacity to my local physician.

I hereby release HomePharma.net and all of its owners, employees, and contractors including physicians from any and all liability whatsoever associated with any adverse effects I may suffer from my use of this medications. I understand the prescribing physician and dispensing pharmacy is an independent practitioner and not an employee or principal of HomePharma.

I realize there are risks as well as benefits to any medications, even over-the-counter medicines. I have been fully informed of the effects, risks and benefits of this medication.

I hereby release HomePharma.net and all of its employees and contractors including physicians from any and all liability whatsoever associated or connected with my Medication Consultation and/or use of the medications.
